Exitcare’s Track My Health project is personalized healthcare, controlled individually and powered by nerds. Built using PHP in Zend framework, the application connects to Exitcare’s MySQL database to put the consumers in control of their records, their history, their present, their future.
The site allows users to keep track of their healthcare history (and that of their family) from prescription drugs taken to doctors visits to medication allergies -- it's all right at their fingertips. Users can create guest cards to share their medical histories with invited guests – like their doctors or child's school nurse, or not, if privacy is preferred.
Plus, there's a section that allows users to research any illness they might be diagnosed with or medication they are prescribed. All the articles are doctor-approved and backed by a verified database of medical research.
